Patient Rights and Responsibilities

At PriMed Physicians, we are committed to providing you with the best possible health care. As such, we encourage you to become an active partner in your health care by learning more about your rights and responsibilities.

I. Patient Rights

  1. Patients have the right to quality services, appropriate to their health care needs, which are delivered in a timely manner.
  2. Patients have the right to care that is appropriate for medical necessity.
  3. Patients have the right to reasonable access to medical care.
  4. Patients have the right to confidentiality in regard to medical and social history, individual medical records, and medical information.
  5. Patients have the right to be treated with dignity, respect, and consideration.
  6. Patients have the right to be informed about personal health as it concerns medical conditions, diagnostic tests, and treatment plans.
  7. Patients have the right to change physicians/ providers.
  8. Patients have the right to a second opinion.
  9. Patients have the right to involvement in decision-making regarding treatment.
  10. Patients have the right to refuse to participate in research. Human experimentation affecting care or treatment shall be performed only with a patient’s informed consent.
  11. Patients have the right to auditory and visual privacy during a visit.
  12. Patients have the right to approve or refuse the release of information except when a release is required by law.
  13. Patients have the right to refuse treatment or therapy. Such persons will be made aware of the consequences of their decision and it may be recorded in their medical record.
  14. Patients have the right to create Advance Directives, which let providers and others know the person’s wished concerning medical treatment.
  15. Patients have the right to assert complaints and grievances about the providers and the health care provided.
  16. Patients have the right to be informed about the role of medical students/ supervised practitioners and the right to refuse such care.

II. Patient Responsibilities

  1. To become fully informed about the extent of your insurance coverage.
  2. To become knowledgeable of the system to access medical care.
  3. To keep all scheduled appointments and to notify the provider when unable to keep a scheduled appointment.
  4. To be on time for all scheduled appointments.
  5. To follow all medically appropriate physicians orders and prescriptions.
  6. To treat all personnel with courtesy and respect.
  7. To provide complete health status information for accurate diagnosis and appropriate treatment.
  8. If possible, to always call your PCP before receiving urgent and emergency care, when mandated by their insurance plan.
  9. To notify your PCP when you receive emergency care within twenty-four (24) hours, or as soon as possible when mandated by their insurance plan.
