Welcome to PriMED Physicians. We are honored you have chosen us to care for you and your family. Our team is focused on building a lasting relationship with you, grounded in communication, trust, and exceptional care. The information below will help you prepare for your first appointment.

New Patient Process

A little preparation goes a long way toward making your first visit calm, comfortable, and productive. Here is what to take care of before your appointment:

  1. Choose Your Provider: Find a care team that feels right for you and your family. We make it easy to connect with a provider who fits your needs.
  2. Schedule Your Appointment: Call your preferred PriMED office during regular business hours. Let us know you are booking a new patient exam and mention any specific concerns so we can plan accordingly. We do our best to see patients with urgent needs within 24 hours.
  3. Fill Out Your Paperwork: Complete your forms ahead of time to save time at check-in and help streamline the registration process.
  4. Confirm Your Insurance: PriMED participates in most major insurance plans, including Medicare. Contact your insurance company directly to confirm PriMED is in your network before your visit.

All new patients are required to call our office to schedule their first appointment. This gives our team the chance to gather your information, answer your questions, and match you with the right provider for your needs.

Once you are an established patient, you will gain access to our online MyChart portal <link to patient portal page>, where you can conveniently schedule future appointments, message your care team, view results, and manage your health information.

Please note that all pediatric patients must call to schedule their appointments. Online scheduling is not available for pediatric visits, so we kindly ask that you contact our office directly to book your child's care.

New Patient FAQs

What should I expect at my new patient appointment?

From the moment you arrive, our team is here to make your visit as smooth and welcoming as possible. Here what to expect for your first appointment:

  1. Arrival and Check-In: Plan to arrive 10 to 15 minutes early. Bring your insurance card and be ready to confirm your contact details. We may take a photo to verify your identity.
  2. Medical History Review: You may be asked to complete a health history questionnaire covering past care, medications, and general wellness. Your provider will review it with you at your visit.
  3. Your consultation: Your first visit is all about getting to know you. Your provider will review your medical history, discuss any current health concerns, and work with you to create a care plan tailored to your needs. We want you to leave feeling heard, supported, and confident in your care.
  4. Payment and Co-Pays: Co-payments are due at the time of service. We will file your insurance claim. Uninsured patients pay at the time of the visit. For pediatric visits, financial assistance may be available. Visit childrensdayton.org<link> or call 1 (800) 228-4594 to learn more.
  5. Schedule Your Follow-Up: Before you leave, book any follow-up visits needed to stay on track with your care.

What should I bring?

Please bring a valid photo ID, your insurance card, a list of any current medications, and your completed new patient forms. If another provider has performed any recent tests or imaging, bringing those results along will help us care for you more effectively.

Do I need to fill out forms before my appointment?

Yes. We ask that you complete your new patient forms before your visit. This simple step helps speed up the registration process and allows more time for you and your provider during the appointment. You can find all the necessary forms in the New Patient Forms section above.

Can I schedule my new patient appointment online?

New patients are required to call our office to schedule their first appointment. Once you are an established family practice patient, you will be able to schedule future visits online through MyChart.

Please note that pediatric appointments cannot be scheduled online. All pediatric scheduling must be done by phone.

What can I expect after my first appointment?

As an established patient, you’ll have secure access to the MyChart patient portal after your first visit, where you can request appointments, ask for prescription refills, send non-urgent messages to your care team, and view your health records. If you are setting up the portal for the first time, our front office team is glad to walk you through it.

For urgent medical needs outside of regular hours, a physician is available around the clock by calling your office number and following the on-call instructions. Please keep in mind that routine refill requests are handled during regular business hours only.

When your care requires a specialist or additional services, we work closely with a trusted network of diagnostic facilities, physicians, therapists, and community resources to make sure all aspects of your health are covered.
